I have two mailboxes setup in Apple's Mail program. One is my .Mac mail
the other is my office mail on an Exchange server to which I'm
connected via the Cisco VPN client.

Mail is set to check my mail every 30 minutes, which it is doing for
the .Mac mail, however it never seems to check the office mail. I only
find new mail in that account when I click on the "get mail" icon -
then the new mail arrives.

What gives?

go to Mail->Preferences->Accounts, click the "Advanced" button, select
the account in question and check the "Include when ..." button (and any
others you might want).

Excellent thanks, I had missed that option!
Must have been something that I had turned off in the past as most of
the time I keep my office mail "offline".
