Title bar mystery

In placing an application from my desktop into my Applications pane, I
dropped the app into the title bar rather than below it where it should
have gone.

The app showed up below in its proper place, but an icon was left in the
title bar; it could not be dragged away.

I then deleted the app, and the icon disappeared with it.  But where the
icon was, there is now a big question mark.

What have I wrought, and how do I get rid of that question mark?

Command+drag it off.

You didn't really do anything wrong; it's supposed to work
that way.
