I understand websites visited will be stored in the history folder.  Is it
possible for websites to be stored to the folder without actually visiting
the site (i.e. pop ups blocked, embedded files from a visited site, etc.)?

It looks like we have websites listed in our folder that when clicked show
sites we have never knowingly visited.

Well I would assume that the corresponding windows had to be opened at
some point (and yes, popups might count in this regard).
