I agree, it sounds like a sense line problem, the Mac thinks that it is
connected to a monitor that can only support 640x480, try a different
monitor, or if you have a monitor adaptor plugged in (D15 to HD15) then
check it's switch settings, if it is an unswitched one then bin it and
get one with switches.

So...
Not sure how this "fixed" it, but...
I plugged my older Apple LCD monitor into the ADC port (DVI? Who can
remember the acronyms...)
Anyway -- *that* worked. I had all the resolutions.
So, I shut down again, plugged the CRT back in -- same monitor -- same
cable (no bent pins) and restarted again.
Magically, all the resolutions were there for the CRT. And I *had*
plugged and replugged the VGA monitor in dozens of times...
Go figure.
