That's odd. I'd suggest he try rebuilding the Settings file and see if
that corrects the behavior. Also, I wouldn't trust the attachments to
stay in the spool folder--he maybe should copy them to another location.
To rebuild the Settings file: hold down all modifier keys
(command-option-control-shift) while selecting Settings from the Special
menu. Some of his customs settings will be retained (server names, user
names) but some will revert to default settings. To make it easier to
reenter his preferences, he can get a list before rebuilding the
Settings file by opening a new message and then selecting "Insert System
Configuration" from the Help menu.
Let us know if this fixes the peculiarity, okay?

I found a similar issue today with the M$ version. If you have
Content Concentrator on, you get the text line at the end of the
document. 'Blah-Blah-Blah' gets you the icon back. No sign of the
attachments being moved, however...
