Instructions I saved from another forum some time ago:
You need to rebuild the address book. Hold down the option key while
opening the address book from the Window menu.
If that doesn't work, you need to edit the resource fork. Here is a
quick and dirty way to do it.
1) Stop Eudora.
2) Select the Eudora Settings file and go CMD-D to dupe it.
3) Open Eudora Settings with a resource editor such as ResEdit or
ResFool.
4) Open the STR# Resource.
5) Select ResourceID 610 and hit delete.
6) Use CMD-K to create a new Resource
7) Use CMD-I to renumber it from 128 to 610
8) Close Window.
9) Save file and quite ResEdit.
Now start up Eudora and there should be NO Quick Recipients any more -
They lived in STR# ResourceID 610. NOW use the Rebuild to repopulate the
list. Now look at the list to see if you have only the entries you
wanted.
If this does not work, then you can ALWAYS just quite Eudora, delete
Eudora Settings, and rename the Eudora Settings Copy created in Step 1
Above to fall back to where you were.
Note: In lieu of steps 5-7, you can just OPEN ResourceID 610 and edit it
to delete the entries you do not want.

It sucks that Eudora bugs me with this problem only once every 5 years.
Each and every time I'll by then have forgotten this solution.
I recently fixed two such 'ghost' entries by deleting enteringthe exact
same nicknames anew, save the Address Book, then deleting those new
entries (and save again). (Funny that it allows you to add a new entry
that it considers to exist already :))
